How do I make an appointment?

You can make an appointment for a counseling session by calling Michelle Kearby at 469-236-9795. Please leave a message and you call will be returned within twenty-four hours. If you feel your call is an emergency, it is advised that you call 911 or proceed to the nearest emergency room.

Is payment due at the time of service?

Yes, payment is expected at the time services are rendered.

Can I use my health insurance?

The client is asked to self-pay and file with his/her insurance company for reimbursement. Responsibility for payment rests with the client.

What is the cost of your services?

The fees for counseling are $100.00 for a fifty minute session. The fees for the initial consultation is $150.00 and lasts approximately two hours.

Is my information confidential?

All information a client shares in counseling, including case notes and records, is confidential. No information will be disclosed without the written permission of the client or a legally authorized person (i.e. parent or guardian). This policy does not apply in the following situations:

1. As part of the counselor's professional development, the counselor regularly seeks clinical consultation and may discuss information about the client's case with another professional counselor who maintains the same policy of confidentiality indicated above.
2. If the counselor becomes aware that the client may be a harm to themselves or someone else, of any suspected abuse or neglect of a child, elder, or dependent adult, of a situation involving stalking, or of a threat to national security, the counselor is required by law to report this information to a designated agency.

What is your cancellation policy?

Clients will be charged the full fee for failing to cancel an appointment within twenty-four hours. In cases of emergency or special circumstances where twenty-four hours notice is not possible, the late fee may be waived.
